1) APPLICATION FORM - Copy I
The IDP application must be complete and accurate. Please send Copy I of the application, along with 2 passport size photos, to:

2) APPLICATION FORM - Copy II and APPLICATION FEE
A non-refundable application fee of US $150 (money order or cashiers check) must be submitted with Copy II of the application, to:

3) OFFICIAL TRANSCRIPTS*
Original dental school transcript and diploma or notarized photocopy of both. (Notarized copies must bear original notary stamp.) Applicants invited to interview/testing session will be required to present original documents at that time. Original non-dental school transcripts from every college or university attended.

4) ECE REPORT
Original ECE course-by-course credential evaluation report of foreign dental education mailed directly to UCSF by Educational Credential Evaluators, Inc. (ECE).

To ensure uniformity of evaluation, the ECE is the only credential evaluation report accepted. The ECE website can be found at www.ece.org.

must
request a course-by-course evaluation and
must
check under section 3 in the application (Purpose of Evaluation)
both
for Further Education
and for ADA-Joint Commission on National Dental Examinations.

5) PART I of the NATIONAL BOARD DENTAL EXAMINATION
Test results of Part I of the National Board Dental Examination must be submitted by the administrators of that examination directly to the UCSF Office of Dental Admissions. The examination must be completed with an overall average of 83 or above and a minimum of 75 or above on each component.
6) ENGLISH LANGUAGE PROFICIENCY- TOEFL
Test results of the Test of English as a Foreign Language (TOEFL) must be submitted by the administrators of that examination directly to the UCSF (institution code is 4840; department code is 38). The minimum acceptable score is 587 if paper-based, or 240 if computer-based, or 95 or higher if iBT. WAIVERS WILL NOT BE GRANTED for scores below the stated minimums.
7) LETTERS OF RECOMMENDATION (in English)
Three original letters of recommendation are required: one letter must be obtained from the Dean or head official of the applicant's dental school, a second letter from a former dental school instructor able to evaluate the applicant's potential for the IDP, and a third from a person of the applicant's choosing, preferably someone with whom the applicant has worked. Letters must be written on letterhead stationary providing address and contact phone number.
8) CERTIFICATION OF FINANCES
Before final admission, qualified applicants who need a visa must complete a Confidential Financial Statement. The certification is necessary for UCSF to provide a Certificate of Eligibility (I-20A/B) which allows the applicant to apply for a student visa. The I-20 A/B is only given to applicants granted final acceptance to the IDP.

9) CURRICULUM VITAE
Applicants are required to submit original, official, certified English translation of ALL documents that are not prepared in the English language. Photocopies are not acceptable. Each translator must provide an original declaration with each translation attesting his/her fluency in the particular language and certify under penalty of perjury that the translation is complete and accurate to the best of his/her ability.

The School of Dentistry recommends, but does not require, that applicants with non-English academic credentials use one of the following sources for translation:

1) Translated/notarized by applicant's school
2) A translator accredited by the American Translators Association (ATA): website: www.atanet.org, phone: (703) 683-6100 or e-mail: ata@atanet.org.
3) Certified or registered court interpreter. Information on court interpreters is available through the Judicial Council at (415) 865-7530. General information is provided at the website, www.courtinfo.ca.gov. The court interpreter must sign the translation and declaration in the presence of a Notary Public.

Applicants who complete Part I of the application procedure and show potential for success in the program will be required to participate in Part II, which will take place at the UCSF School of Dentistry. Instruction manuals will be provided prior to the Technical Skill Examination. Part II will consist of:
1) Basic Dental Assessment Test (Written Test)
2) Technical Skill Examination (Technical Exam Fee is $200)
3) Personal Interview
*Any notarized copy sent to the dental school should bear the original notary stamp.

4) Have you ever received any awards for outstanding academic/scholarly achievements, athletic/musical talent, or community service? Please list and explain.
5) Please list any publications for which you were an author and research projects in which you participated.
6) Are you proficient (written or conversational) in language(s) other than English? Please list them.
7) If you observed a classmate cheating, what if any action would you take and why?
8) After receiving your DDS, how and where do you envision your skills being employed?
9) Please state briefly why you should be selected for admission to the UCSF IDP over other applicants with similar qualifications.
