| Endodontic specialty clinic providing the following services: nonsurgical root canals, nonsurgical root canal retreatments, and apicoectomies. A comprehensive exam will be performed at the beginning of every appointment to diagnose orofacial pain and/or pulpal infection to determine whether the tooth is a good candidate for endodontic treatment. Providers include American Association of Endodontic members: Frank Crawford, DDS, MA; Jennifer Gibbs, DDS, PhD; and Tri Huynh, DDS, PhD. |

| The clinic accepts a wide variety of insurance plans and is a PPO provider for Delta Dental. We will assist with completing and submitting forms electronically and/or by paper. |

| Your first visit is a screening appointment
only. You will not receive any treatment on this visit unless
you need a referral to an emergency clinic. There is no charge
for a new patient screening appointment. |
