Equipment Loss

Educational Support Services has implemented a claims procedure for lost or stolen dental equipment. Although each class is responsible for administering its own instrument loss plan, the following procedure should be followed:

Student reports the theft or loss to the UCSF police on MU G level.

Student submits a copy of the theft report to Educational Support Services, D-4010.

Student reports claim to the Class Treasurer.

Treasurer obtains claim information and instructs student on how to proceed, including forwarding of the police report.

Student orders replacement equipment from the school’s dental supply area.

Student advises Treasurer when equipment has arrived at the supply area and amount of replacement cost.

Treasurer mails payment to the student by draft. In turn, the student writes a check to UC Regents in payment for the instruments from the supply area.
