Absence Protocol
In general, excuse from class, clinic or rotation is at the discretion of the appropriate faculty member. The following procedures are outlined to provide direction for students who must be absent from a class, clinic or rotation.
Absences
All Absences
* Call Educational Support Services (476-1101) to report your illness or absence.
* If your instructors require you to document your absence, request a Student Absence Record from Educational Support Services in Room D 4010.
* When appropriate, visit Student Health Services or other health care provider for treatment of illnesses or other health problems. Documentation from your health care provider is not required for brief illnesses.

Patient Care Activities
* Call your scheduled patients to cancel or reschedule their appointments.
* Call the Office of Clinical Services (476-1778) to inform them of your absence and that you have contacted your patients to cancel or reschedule. The Office of Clinical Services will inform additional faculty or staff of you absence, if needed.
Didactic and non-clinical courses
Consult your course outlines for policies on attendance and makeup work for the specific courses you are taking. If you will be absent from an examination it is critical that you contact the appropriate faculty member directly before the examination is scheduled to begin. (Students who are “no-shows” for an examination without an adequate reason may receive a failing grade.)
Longer-term emergency absences
Follow the protocol outlined above, but for illnesses or absences of longer than two consecutive days, you should provide written documentation (e.g., note from physician, etc.) to explain your absence.
Planned absences
Planned absences may include medical appointments, jury duty, court appearances, family emergencies, etc.
Patient Care Activities
Complete an Administrative Action Form, have it signed by your course director, and return it to Network Information Services in room D 1000 as soon as possible or at least one week prior to your planned absence. You may obtain an Administrative Action Form from the Department of Preventive and Restorative Dental Sciences, Student Services, Room D 3215.
Didactic and non-clinical courses
Complete a Planned Absence Authorization Form and turn in the form with documentation of your planned absence to Educational Support Services in room D 4010.
Dental Care
The School of Dentistry has designed a package of basic preventive care, delivered in two or three conveniently scheduled appointments. The package includes: dental exam; four dental x-rays, teeth cleaning; sealants, as needed; oral health counseling; nutritional counseling; dental care kit. All care is performed by second year dental students, supervised by Dental School faculty. Patients with more serious dental problems will be referred to the UCSF Comprehensive Care Clinic or another dentist. UCSF follows all current infection control standards. Evening appointments are available to fit around your busy schedule. Our special package fee of $50 compares favorably against $125 or more for care in a private setting. Insurance and Denti-Cal are accepted. Students receive a 50% discount on dental services (with Clinic Director approval). For more information call 415/476-1891.
