Your next steps as an admitted student
Complete the steps below to secure your seat in the program and finalize your admission. Each item has specific requirements and deadlines, so please review them carefully.
-
To officially accept your offer, submit a $1,000 deposit (check or money order payable to “UC Regents”) by July 29, 2025. Mail your payment to the address provided on the form. Do not send any payments to the School of Dentistry or the Office of Admissions & Outreach.

Please submit a disciplinary background check to all colleges and universities you will have attended prior to matriculation at UCSF. This is required of all admitted applicants. This process must be initiated no later than 2 weeks of acceptance. Failure to do so will result in your offer of admission being rescinded, even if you already have submitted the non-refundable deposit.

Within 3–5 days of receiving your offer, Certiphi Screening will email you instructions for completing your required background check (add studentedition@certiphi.com to your safe-senders list). This background check is required for all admitted students.
You must begin this process within 15 days of the date on your Provisional Admit Letter. Failure to initiate the background check on time will result in your offer being rescinded, even if you have already paid the non-refundable deposit.
If you recently completed a Certiphi screening for another institution, UCSF will have access to it and no further action is needed.
-
Review the technical standards required for successful completion of the DDS program. By signing the agreement, you acknowledge these standards and understand they will be used to evaluate your professional behavior and competence throughout the program.

Before patient care begins, you’ll learn essential techniques in the simulation lab using extracted human teeth. While plastic teeth are used for some exercises, many procedures require real teeth, and UCSF cannot supply the number needed for training.
Please begin collecting extracted teeth from local dentists as soon as possible. Oral surgeons and general dentists are typically familiar with dental-student needs, but you may need to contact multiple practices, as students from other programs may also be collecting.

This form is used to verify your ability to cover the entire cost of your education and living expenses. This verification is required before your I-20 can be issued as part of your visa application. This form is using the 2025-2026 costs to get the process started; however if the fees change you will have to update your information if the total funds do not cover the expenses.
